What forms are required for camp and how do I submit them?

All families should be prepared to submit the following documents:

• Record of a child’s physical within the past 18 months for summer camps (within the past 12 months for afterschool).
• Child’s immunization record or signed record of exemption.
• Child’s medication history, current medications, and – if requesting medicine be administered during camp – physician’s medication orders 

As a camp registered with the MA Department of Health, we are required to collect information for the health and safety of our campers. Please complete your Sawyer profile fields soon after you register to ensure we have the materials we need to admit your camper!  These documents MUST be received at least 3 business days before the first day of camp in order for us to have time to review these documents.
